The University of Osaka Dental Hospital have the Fundamental Principles of “Contribute to the advancement of oral healthcare through the promotion of education and research in oral medicine via medical treatment.” In terms of quality and safety management, the most basic and important issue is to provide safe, secure, and advanced medical care to patients and their families. The basic policy regarding the safety management of medical care at our hospital is as follows.

1.Basic Approach to Patient Safety Management

We strive to create a system that allows patients and their families to receive medical care with peace of mind. We achieve this by sincerely listening to their needs, providing sufficient and accurate information, and ensuring they feel comfortable and understood throughout their treatment process. We actively encourage their participation in their own care. To ensure patient safety, we promote awareness among our staff and strive to prevent medical errors while maintaining a high standard of medical care. All members of our hospital work together to promote patient safety.

2.Organization for Quality and Safety Management

We have established the following organizational structure:

(1) Division of Quality and Safety Management

This division is responsible for investigating and analyzing patient safety issues, disseminating guidelines, confirming and advising on the management of patients and their families when medical errors occur, confirming and advising on the content of medical records and nursing records, and investigating the causes of incidents.。

(2)Quality and Safety Management Committee

This committee plans and develops patient safety management measures and measures to prevent medical errors, creates manuals, plans and manages education and training on safety management, investigates and analyzes reports of accidents and incidents, coordinates with risk managers, confirms and advises on the status of descriptions of medical errors, etc., in medical records and nursing records, and takes measures to prevent medical errors.

(3)Risk Manager

Each department has a risk manager, who is responsible for disseminating information on patient safety management measures and measures to prevent medical errors to members of the department, providing guidance to encourage the active submission of accident and incident reports, and working to ensure patient safety and prevent medical errors as a member of the Quality and Safety Management Committee.

(4)Risk Checker

We have several risk checkers who periodically patrol the hospital to verify that quality and safety management measures and measures to prevent medical errors are being properly implemented in each department.

(5)Monitoring Committee Member

We have several monitoring committee members who monitor incident reports, analyze the causes of incidents that occur in the hospital, and propose measures to prevent medical errors.

(6)Person Responsible for the Safety Management of Pharmaceuticals

The person responsible for the safety management of pharmaceuticals verifies that pharmaceuticals are being used safely and appropriately.

(7)Person Responsible for the Safety Management of Medical Devices

The person responsible for the safety management of medical devices verifies that medical devices are being used safely and that maintenance inspections are being carried out appropriately.

3.Training

We hold lectures, education, training, and implementation training sessions on quality and safety management for all staff and students at least twice a year to raise awareness and promote the prevention of medical errors. All members of our hospital are required to participate actively in these training sessions.

4.Improvement Measures for Ensuring Patient Safety

Accident and incident reports that occur at our hospital and are deemed effective in preventing medical errors are featured in our in-hospital newsletter, "Incident Report News" (published monthly), to help prevent medical errors.

5.Response to Medical Errors

In the unlikely event that a medical error occurs at our hospital, we are committed to responding in accordance with the "The University of Osaka Dental Hospital Qality and Safety Management Manual." Our response to such an event will be grounded in sincerity and transparency towards patients and their families. We will, of course, take all necessary medical measures. Equally important, we will provide a full and honest explanation of the facts and ensure transparency in all aspects of our response.
